Generate some ideas at one of the levels, and transmit them around both horizontally - to other groups - and vertically - via the leaders, to another level. At the upper level, vote on whether the proposed idea merits further action, or conflicts with existing principles which make it not desirable.
Send the result of that decision back down for consideration by the originating level. They than have the chance to override the decision, and in doing so possibly considering throwing out their leader, or accepting that the proposal does not work, for the reasons determined by the upper level.
Maybe this sounds complex, but I suspect that once the ways of doing this are embedded, the actual mechanics are pretty easy to manage. At least with this approach, all interested parties at all the levels are seen to be engaged in the process, so we get none of the powerlessness effects of the existing systems.
Hopefully this may also cut out some of the problems of making decisions which subsequently see us making very expensive mistakes, and creating even more expensive enquiries into the disasters.
So much stuff I see as being reactive to problems, rather than proactive in creating the correct solutions first. I am well aware that its easy to be critical after the event, but we must generate some way of avoiding the necessity for hindsight.
